Medication Reviews

Get the most out of your medication

A medication review helps to ensure the medications you’re taking are safe and effective for you. Your pharmacist will review all of your medications, make sure you know how to take them properly, and discuss possible side effects or interactions. For example, your pharmacist will let you know if there are risks in drinking alcohol or taking certain herbal supplements or vitamins along with any of your medications.

If your pharmacist is concerned that a medication may not be appropriate or thinks another medication might work better for you, they’ll share this information with you and your primary healthcare provider. In some cases, based on the provincial scope of practice, your pharmacist may be able to make certain changes or adjustments independently.

Who should get a medication review?

Anyone can benefit from a medication review. They can be especially helpful if you:

  • Take more than one prescription medication
  • Have concerns about the side effects of your medication
  • Are elderly
  • Have a chronic health condition
  • Have had a recent hospital stay

Are you a family member or a caregiver who has concerns about the medications someone else is taking? With their consent, you may ask for a medication review on their behalf.

How can I prepare for a medication review?

A medication review is a one-on-one consultation with your pharmacist. To get the most out of your review, bring your current prescription medications, as well as any over-the-counter medications, vitamins, or supplements that you take. Write down any questions you have ahead of time.
